This study of myopathy in rats leaks quite a bit of damaging information, not the least of which is that statins can induce myopathy!

Rosuvastatin is a relatively new member of the statin family (HMG-CoA reductase inhibitors), with superior lipid-lowering effects and a pattern of clinical side effects, including a low incidence of myopathy, similar to other widely prescribed statins. This article describes investigations of myopathy in the rat following administration of very high doses of rosuvastatin. The nature of the changes were found to be entirely consistent with those seen with other statins, including a differential sensitivity of muscle fibers (with glycolytic fibers [type IIB] the most sensitive and oxidative fibers [type I] the least), a delay of approximately 10 days after the start of oral dosing before necrosis was apparent, and ultrastructural alterations appearing first in mitochondria. In addition, the development of myopathy was prevented by coadministration of mevalonate, the product of HMG-CoA reductase. The findings illustrate a pattern of induced myopathy in the rat directly attributable to inhibition of HMG-CoA reductase that is entirely consistent between the various statins, with the oral dose required to produce the changes being a differentiating feature (based on these new data and a previously reported study from the same laboratory): cerivastatin dose less than simvastatin, and simvastatin dose less than rosuvastatin.

Effect of atorvastatin on left ventricular diastolic function and ability of coenzyme Q10 to reverse that dysfunction

This study evaluated left ventricular diastolic function with Doppler echocardiography before and after statin therapy. Statin therapy worsened diastolic parameters in most patients; coenzyme Q10 supplementation in patients with worsening diastolic function with statin therapy improved parameters of diastolic function.

Dose-related decrease of serum coenzyme Q10 during treatment with HMG-CoA reductase inhibitors
Coenzyme Q10 (ubiquinone) the essential mitochondrial redox-component and endogenous antioxidant, packaged into the LDL+VLDL fractions of cholesterol, has been suggested as an important anti-risk factor for the development of atherosclerosis as explained by the oxidative theory. Forty-five hypercholesterolemic patients were randomized in a double-blind trial in order to be treated with increasing dosages of either lovastatin (20–80 mg/day) or pravastatin (10–40 mg/day) over a period of 18 weeks. Serum levels of coenzyme Q10 were measured parallel to the levels of cholesterol at baseline on placebo and diet and during active treatment. A dose-related significant decline of the total serum level of coenzyme Q10 was found in the pravastatin group from 1.27±0.34 at baseline to 1.02±0.31 mmol/l at the end of the study period (mean±S.D.), P<0.01. After lovastatin therapy the decrease was significant as well and more pronounced, from 1.18±0.36 to 0.84±0.17 mmol/l, P<0.001. Although HMG-CoA reductase inhibitors are safe and effective within a limited time horizon, continued vigilance of a possible adverse consequence from coenzyme Q10 lowering seems important during long-term therapy.

Owen, do you know how the patients are receiving the CoQ10 for this study? is it by mouth or by needle?
I ask because I think Dr. Langsjoen said that ubiquinone (CoQ10) is difficult to absorb when taken orally vs ubiquinol which is much more absorbable.
